Submit electronically to the ESDS office by emailing all documents to blanka@evelynstreetdayschool.com.  You can choose to email them as a zip file or as individual documents. Ask your pediatrician to fax your child’s Universal Health Record to 201-523-9559. All documents have to be submitted prior to your child’s first day.
